* Any student that will be taking medication throughout the year needs to have the appropriate forms filled out. To acquire the appropriate forms please contact me in the clinic.
- Students should not carry any medications to and from school unless a doctor has given permission and I have been notified and I have the appropriate forms on file.
- Please be sure to notify me if your child’s health status changes in any way during the school year. Such as: Asthma, Allergies, or any other serious health conditions requiring assistance.
